We request that you bring your current insurance card to each office visit. Please be prepared to pay your co-pay at the time of service. We accept all major credit cards, checks or cash.

Some insurance companies require a referral from your primary care physician before you see a specialist or have some types of medical testing. If your insurance requires a referral, you can call the office at 941-365-1321 and choose option 4. You will need to leave your name, the name of the specialist, and the date of your appointment. We ask that you give us 48 hours notice for all referral requests.

Appointments can be scheduled by calling the office at 941-365-1321 and press option 5 for the receptionist.

You can obtain refills by calling your pharmacy and requesting the pharmacist fax our office the refill request. Requests for all other prescriptions, including written prescriptions for mail away pharmacies can be called to our prescription line at 941-365-1321 and press option 3.

When you call the office, remember to have the name and the strength of the medication and the pharmacy name and phone number. We ask that you allow 48 hour notice for all requests.

If you would like copies of your medical record, submit those requests in writing to our office. If you would like to leave a message, you can call our office at 941-365-1321 and press option #3 If you need copies of recent testing or office visit notes, leave a message at the office and the records are available for pick-up at the front desk. We request 24 hours notice for any re-quest for pick-up.

Our physicians provide outpatient services in our medical office. If you need admission for in-patient hospital care, the physicians see patients at Sarasota Memorial Hospital.
